When I posted the question about jack field construction, I did not realize that I had the solution in my own study:

It is the Naval (possibly A/N) teletype patch panel. Since I have WE jack strips in my 19" radio rack to connect to individual TTY units, I will install a shelf in the rack unit for this device. I have a Teletype loop power supply, I will use that to power this.
I want to thank everyone for their suggestions and ideas.
