For those who asked me for a copy see the link above.  It's a different scan than mine, with color photo on the cover instead of B&W, and with some pages in a different order.  From that, I conclude that it is scanned from a different copy of the document.  However, the handwritten page numbers are all identical, as are many of the speckles on the pages.  From that, I conclude that these are artifacts of photocopying that was done before the two different independent scans were made.
