[GreenKeys] ASR-33 not punching all the way

I'm in the home stretch of my ASR-33 restoration. Everything is working
great now, except the punch.

I'm getting "hanging" chads on the right-most three bits (as seen from
the front). The punches will go half-way through the tape, but not
cleanly remove the chad, so the tape gets stuck. (I have already
lubricated everything according to the manual)

I tried to understand the Tape Punch adjustment guide in the ASR-33
manuals, but I don't think I'm following it very well. In particular, I
think the Punch Penetration (from Secion 2.05 of the manual) is out of
alignment. Is there anything else I should look at?
