[GreenKeys] 14TD with chadless tape

Well that was quite the outpouring of help! As it turns out, it looks like the
feed roller was slightly misaligned, causing the feeler pins to come up right at
the very edge of the tape holes and sometimes catch on the edge of a hole. 
For reference, the adjustment is described on the 141B PDF pg 26 (page 2-17, 
paragraph 32) and just involves repositioning the detent roller bracket a tiny
bit. It looked like someone had pushed it off to one extreme of its range, but
after adjusting, the feeler pins always hit within the holes and I'm getting 
100% error-free reads.. whee! Thanks for all the helpful advice!
