You are a good investment that I made by sending you that RT.
Most of the equipment I labored over from 1984-2002,
and spent over $14,000 on, is collecting dust behind a
locked door in San Diego...

Many thanks to folks here for help and encouragement.

I'm mainly writing this stuff up for my own notes, but also in case
someone goes searching the archives looking for info on a similar
problem. So yes, the punch slides can wear and this leads to improper
punching. There is felt wick attached to the slide post (the part that
engages the slides to lift them and their associated punch pins), but
it is deep inside and may not get oiled as it should.

In reassembly, my chief issue was getting the perforater drive arm
rocker correctly re-connected to the toggle shaft that wiggles the
toggle bail and toggle link. I didn't find this info in the
adjustments section of the manual I have (perhaps I just missed it),
but it is critical in order for the punch slides to engage the slide
post properly when unlatched and to get re-latched by the reset bail.

OK, I'll be quiet for a while - next up is getting the TD running on  
this 28RT.
