[GCARC] NJ QSO Party question about summary sheet

I was reading the rules for the NJ QSO Party.  The logs section states "A summary sheet (see website) must be submitted in addition to the Cabrillo file.".  I do not see anything defining the summary sheet format.  Anyone familiar with what the sheet should contain?
