1) I would suggest using the demo version of Circad to draw up your schematic and 
PCB http://www.holophase.com   Either DOS or Win versions.   It's limited but I 
think it will handle a small project like that.

2) Advanced Circuits has some good prices on PCBs.  They have an online "quote" 
program on the web page which will give you your cost.  They also have some 
"deal" going for 3 PCBs etc that might be of interest.  The card they produce are the 
nice double sided (With plated through holes) soldermask, silk screen etc.
