Jim,

PSK31 is continuous duty transmission, and the 20 watts idling is also 20 
watts transmitting.  The heat sink will get quite warm with longer 
transmissions, but should not be so hot you cannot keep you hand on it.

Adding the muffin fan would be add extra comfort range even if it is not 
required.  It can just lay on top the heatsink, moving air upward.  If it is 
a DC fan, you can put a resistor in series with one power lead and slow it 
down considerably for less noise.
