When I was putting mine together I had this problem too, took me a while to 
find it. When elecraft countersink the screw hole it can leave a little 
tiny piece of swarf, spotted it eventually ;-)
the shortness of the shoulder washer isn't a problem.
I also stuck the pad to the transistor not the case as it made assembly easier
