Sean,
You apparently have some problem with the 40 meter Bandpass Filter.
Look carefully at the two inductors (L1 and L2 as I recall) and the
capacitors in the area.
You should have had 40 meter receive when you did Alignment and Test I - if
you had no problems at that point, ask yourself what components were added
in the bandpass area after that.
After checking component values - check, check and re-check the soldering.
Look for nice looking solder joints with a minimum of solder. Also look
carefully for solder bridges.
Since Elecraft uses thru-plated holes, you can use solder wick to suck away
much of the excess solder from any joint and still have enough solder to
connect the component lead to the circuit board land - in other words, you
do not need a 'nice solder fillet' with plated thru holes.

Problems again.
I have no output or receive on 40m (rx is very deaf) pre amp appears not to
work on any band.
o/p on all other bands no problem.
Ideas??
