It'd make sense that they couldn't pre-approve; My understanding is that
in order for an operation to be approved they have to meet the criteria
and submit the paperwork. If they don't have the paperwork in order, it
doesn't get approved.
I'm sure from some legal standpoint blessing an operation before it
happened and then later having to back out on that would cause a problem
in this society of lawyers.
