[Dxbase] LOTW Upload Continuing

I am continuing to try and upload an ADIF file to LOTW.  I generate the 
file in DXBase, name it and I get a prompt from TQSL confirming signing 
using my call.  However, when I click "yes," I am asked for the date 
range, it says I can leave them blank, and I have.  So, I click OK and I 
always get:

Error:  Certificate QSO date out of range on line 12
Call:  FM5WD
QSO_DATE: 20100313
TIME_ON: 212959
FREQ: 18.07380
BAND: 17M
MODE: CW

This error is always the last QSO in my log, whatever it might be:

I am then asked, ignore errors?

I click "yes."

Then I get "No records output."

Anyone have any idea what I'm doing wrong??????  I have tried this about 
50 times yesterday and today.  At one point I was able to upload a file 
but it only contained five QSOs.  There should be several hundred.  I'm 
lost.
