[Dxbase] PSK help

In opening the DXBase PSK program I have somehow fouled it up and 
keep getting an error message, "Invalid Number of Parameters."  So 
far I am unable to determine which invalid parameters I have created.

Could someone help and tell me how to reinitialize this program or 
set it back to it's original values so I can get a fresh start at 
setting it up.
