[Dx4win] Identity QSOs missing from LoTW

Hi Folks,

I've been working on a new application note to help you identify QSOs
from your log that you *think* have been uploaded to ARRL Logbook of
the World, but really are not:

I would appreciate if a couple of people could look this over and
actually try it, to let me know if any of the information is incorrect
or confusing.
