Tony,
I'm not so sure about your "Golden Rule." This presupposes that the DX
station in question recevied the original QSL request in the first place,
and as we all know, mail gets lost, misrouted, or stolen all the time.
For example, it took me three requests to get my card from a particular A7
station who's starting to get a bad rep. But I found out that he never
received the first two, and the third one only got through after I went to
great pains to disguise it as a routine letter from a business. And there
have been many other times in the past where I've had to make a second
request, after a reasonable period of time, only to find that the first one
never arrived. Couple this with incorrect or outdated address or manager
information on-line (I'm right now looking at a particular CQ WPX 02 QSL
request returned to me by a stateside station who is NOT the manager, even
though he's listed as such in half of the on-line resources -- even
today)...
No, I think if you need the QSL badly enough, a second request is usually
warranted. But a need for a third request may require more consideration
before sending it off.

Never send second requests.
If he didn't answer the first one, he won't answer the second.
If he is slow to answer, you give him twice as much work to do.
