John -

1.  Just pull.  It sticks to the surgical rubber "donut" behind it.

2.  When you remove the dial skirt, you'll find two Phillips head screws 
that hold the escutcheon to the panel

knobs, and Charlie Talbott has some <pincon at erols.com>.  I don't know of 
anyone offering just the "brights" for these knobs.  Charlie has them 
for Collins and 4 Line Drake.
