The relay in the center underneath plug-in is  P&B   R10-E1Y4-V2.5K.  
Its a 4PDT  with a 48 volt coil.
Available from Mouser, just put 655- R10-E1Y4-V2.5K  in the search 
window on www.mouser.com.  Good house to deal with as they don't have a 
minimum order fee.
