[Drake] fuse lamp in TR-4

Chris -

The CM is Chicago Miniature, a lamp manufacturer.  The lamp number is
just "12" and is made by several manufacturers.  It is available from
several sources,

A type 47 lamp is the same electrical specifications, 6.3V @ 0.15A, so
you could just solder some leads to one of those....
