[CW] methods to increase code copying speed

I'm pretty good at 20+ with solid hard copy.  At 25, I am doing more
head copy but I "take notes" on things I need to remember.  That said,
I'm still working on being solid head copy at 25.  Yes, I do recognize a
lot of words, but at 25wpm, if the word's not in my "cw vocabulary" - I
miss it.  I'm not there yet, but getting closer.

(My practice speed is 25wpm with character speed at 30.)

That's a good question you have asked and I can tell you what I have
been doing.

Over the last 5 years I have been working on code during my morning
drive to work.  I figured some people learn a foreign language while
driving, I ought to be able to work up my code skills.  I use one of
those MFJ code tutor players.  I usually alternate by having it play
words and then call signs.
So after all of this time I am around 27 WPM while driving.  I can
recognize words but I wonder if it is because I have most of them
memorized.

When it comes to real code from a QSO, I still can't copy much over
18WPM.  My main problem is that I just cannot write it down fast enough.
I tried typing it in on a keyboard and I can't do that nearly fast
enough either.

Do you really fast guys out there actually write down what you are
receiving or just get it all in your head?

p.s. it would be nice if when the subject of a thread changes that the
