LastNovember, I searched and queried the ARRL website to see if anyone haddeveloped an event planning form to use when serving outside organizations withcommunication services.  I got a lot of responses indicating that there wasnothing available.  Consequently, I created such a form.
 
Ihave obtained permission from the ARRL and from the authors of an articleincluded with the form to use materials used in the creation of the form.
 
Pleasefeel free to use the form and to distribute it to ARES units and ARRL clubs.
 
Theform is attached.
