Hammond makes a transformer you are looking for.  The transformer suggestion 
below will work but it is not built for audio.  (core is stacked for power 
and not audio).  The Hammond would be better.  http://www.tubesandmore.com/ 
Look for P-T119DA  It is good for 12 watts.  8 ohms to 600 ohms it also has 
a 4 ohm tap.
