That's essentially what I suggested in Excel or Work, but you have to sort the
entries with a command. It does give you more flexibility. You can likely find
an old ('95) Office that will do just fine for $10 or so.
IMO, setting up a 'fill in the blank' form application would simply not be worth
the effort unless you can get some dummy to do the data entry.
