i have scanned the original document for this mod; it is 6 pages in
length, in .gif format; anyone wishing the set emailed may send me a
request.  size will be just under 1 mb, so BE SURE your INBOX can handle
it, as i truly dislike having the scans bounce back to my (ancient) 56K
dialup.  i typically wait for multiple requests before emailing so i can
blind CC, saving a little time - therefore, if you don't receive an im-
mediate response, hold your horses for a little while, you'll get'em.
