A suggestion in the short term is use the Header or Preamble part of 
the ARRL forms, then the body is the ICS form. On the ARRL form, the 
first word of the ARRL message is ICS# where # represents the form say 
ICS213 and the next word on the ARRL form the ICS213 message number. 
Attach form 213 with the information and send the body of the ARRL 
message as the attached message and then the signature info. Yes the 
ARRL form is rather specific but why not use the good parts of it (the 
preamble) and sandwich the other form inside of it. Sort of like 
encapsulated TCP/IP packets.... Word count would be the count from the 
ICS213 +2 for the Form Type and Message Number.

Attaching the ARRL radiogram paperwork to the front will be a simple 
staple job and then you wont confuse those that use ISC213 daily with a 
"Custom form" and you still have your tracking. This may seem like a 
compromise but it is a good simple way of making it work without having 
custom forms that someone will get upset about.
