Here is an example our club just did... Not sure if it is what you are
looking for or not..

Memorandum of Understanding Between the Peoria Area Amateur Radio Club, Inc.
(PAARC) and the City of Peoria Emergency Services and Disaster Agency (ESDA)

I. Purpose

The purpose of this document is to state the terms of a mutual agreement
(Memorandum of Understanding) between the Peoria Area Amateur Radio Club,
Inc. (PAARC) and the City of Peoria Emergency Services and Disaster Agency
(ESDA) that will serve as a framework within which the PAARC may coordinate
their services, facilities, and equipment with ESDA in support of local
emergency communications functions.

II. Recognition

The Peoria Area Amateur Radio Club, Inc. recognizes that the City of Peoria
Emergency Services and Disaster Agency is responsible for coordinating the
preparedness for, response to, recovery from, and mitigation of emergencies
and disasters within the corporate limits of the City of Peoria, Illinois.

The City of Peoria Emergency Services and Disaster Agency recognizes that
the Peoria Area Amateur Radio Club, Inc. is one of many organizations
representing the interests of radio-amateurs in the Peoria area and that
those trained and experienced amateur radio operators can be of valuable
assistance in times of communications need.  ESDA also recognizes that the
PAARC owns and controls amateur radio equipment that assists in the
performance of emergency communications by its members.

III. Organization of the Peoria Area Amateur Radio Club

The Peoria Area Amateur Radio Club, Inc. is a noncommercial membership
organization of radio amateurs organized for the facilitation of the
exchange of information and general cooperation between members, for the
promotion of radio knowledge, for the promotion of fraternalism among
Amateur Radio enthusiasts, to conduct club programs to benefit members, the
Amateur Radio community, and the Public, and to advance the general interest
and welfare of Amateur Radio in the general community.  To this end the
Peoria Area Amateur Radio Club, Inc. has affiliated itself with the American
Radio Relay League (ARRL) as a Special Services Club.  Per the ARRL, these
"groups actively pursue all aspects of Amateur Radio: New Ham Development
and Training; Public Relations; Emergency Communications; School Club
Support; Technical Advancement; Operating Activities; Annual Reports, and
more".

IV. Organization of the City of Peoria Emergency Services and Disaster
Agency

The City of Peoria Emergency Services and Disaster Agency is an agency of
the government of the City of Peoria, Illinois whose express purpose is "the
coordination of emergency services functions which may be necessary for or
proper to prevent, minimize, repair and alleviate injury and damage
resulting from any natural, technological or human causes".  "The ESDA
consists of the director and such additional members as may be selected by
the director and approved by the chief executive officer and the City
Council".  City Code Chapter 7 article II section 7-27

V. Principles of Cooperation

A. The City of Peoria Emergency Services and Disaster Agency agrees to allow
the Peoria Area Amateur Radio Club, Inc. to locate amateur radio antenna(s)
in support of emergency communications on the 150 foot tower located at the
south end of ESDA offices at 3615 N. Grandview Drive, Peoria, Illinois as is
approved by and agreed upon by the ESDA Director.

B. The City of Peoria Emergency Services and Disaster Agency further agrees
to allow the Peoria Area Amateur Radio Club, Inc. to locate amateur radio
equipment in support of emergency communications within the grounds of the
ESDA compound at 3615 N. Grandview Drive, Peoria, Illinois as is approved by
and agreed upon by the ESDA Director.

C. The Peoria Area Amateur Radio Club, Inc. agrees to allow the use of such
amateur radio equipment by licensed radio amateurs designated by the ESDA
Director during times of emergency, severe weather, or other such imminent
or occurring natural, technological, or man-made disaster and its aftermath
for tests, nets, and other amateur radio traffic associated with such
incidents.
