Thanks, gents. I downloaded my copy from the site. The first chapter, "Too
Low an SWR Can Kill You" rings true for me. When I constructed the
shunt-feed for my tower on 160M, I needed a bread slicer variable cap for
the gamma match (to lower the SWR of the feed point) that I knew I had
stored in the crawl space above the garage. I dutifully set up the step
ladder beneath the trap door in the garage ceiling and gingerly climbed off
the top step and into the storage area. After I retrieved the cap, I
carried it in one hand and attempted to step back down onto the top of the
step ladder. Of course, I lost my balance (with one hand otherwise occupied
with the cap), the ladder tilted to the side and I fell six feet onto the
concrete garage floor. No major injury but I had the wind knocked out of
me.
Caveat Amateur.

I see the difference. After all that fuss when I downloaded it I found I
already had it in my technical book archive.
