So far, that is all I've seen too. What I was asking was if the "collective 
wisdom" had seen enough bad audio transformers to come to any kind of 
decision on the mechanism of that failure and if there was a common mode 
of failure. So far, it seems as though the answer to both questions is "No."

Yes. I know where those are: buried under other components, primarily. ;-)
