Mike,

I can't speak for foreign navys, but for USN, if you are referring to the
main battery, the short answer is no. No USN battleship in service from
7/12/41 had more than 12 guns in the main battery. Those with 12 guns were either
12" (Arkansas, carried 6 X 2) or 14" (Pennsylvania, New Mexico and
California Class, carried 4 X 3). The Maryland Class carried 8 16"/50 (4 X 2) The
North Carolina, South Dakota and Iowa Class all carried 9 16"/50 (3 X 3). The
Montana Class was to have carried 12 16" (4 X 3) but according to my source,
none were ever laid down (Cancelled 21/7/43).

So the answer toi your second question is no, no more than 3 tubes per
turret. And to your third, yes, several classes had four turrets and the USS
Arkansas had six.

"Class" means that two or more approximately identical ships were built.
The Class was named for the lead ship. The number of ships in a class varied
from two to six, although in the only six ship class, the Iowa, two were
cancelled and never completed.
