  The product is called Four Star NonShrinkable Grout.
     it is a cementicious product, sold at almost all construction material
     yards.
     First I would use double nuts, one set of nuts on the bolts to run down low
     place the plate on the bolts and level by adjusting those bolts.
     Remove the plate and pour the grout
     you mix it in a bucket with water
     pour it onto the existing concrete and then lower the plate down to
     those properly adjusted bolts.
     Let excess grout squeeze out and for neatness, cut it with a trowel
     along the edge of the plate.
     Slightly bevels out at the base, for water flow.
     This stuff hardens quick, so maybe do a bit of practice with it before
     the final attempt
     you do NOT want any voids in the middle under the plate
     That's how we would set those steel columns in building and get the all
     set at the same heights.
