The ground wire always goes to the ground buss bar. Use the bare wire in the
cable for your ground just like any other circuit.
If you have a 3 terminal 220 outlet there are two hot wires and a ground
wire (bare). There is no neutral connection.
It sounds like you are using 12/3 with a bare ground conductor. In this case
you would use the black as one hot, the red as the other hot and of course
the bare as the ground wire going to the 3rd terminal of the outlet and the
other end connecting to the ground buss bar in the sub panel. Just cap off
the white wire with a wire nut at each end and leave it hang.
You can use 12/2 wire with a ground for this 3 wire 220 outlet. Use the
black as one hot and the white as the other hot but you must mark it with
black tape on each end to indicate that it is a hot wire and not neutral.
If you need a 220 outlet where you need a neutral to run 120 volt equipment
as well as 220 then you would need the 3 wire cable with ground. The red and
black would be the hots and white would be neutral and of course the bare
wire would be ground and again connect to the ground buss in the sub panel.
The neutral wire would connect to the neutral buss in the sub panel.
This system requires a 4 terminal outlet.
