[Yaesu] "bar code from gift box" ?

Howdy all -

I got my FT 897 a few weeks back and now want to send in the rebate 
form.  It says to include the "bar code from the gift box" - would that 
be the strip labeled "UPC" on the outer box?  There are 3 bar codes on 
the outside of the box, the one in the middle is labeled UPC, top is 
labeled "ean / jan" and the bottom one is the serial number.

Thanks for any help.  I can use the $200 for a CW filter :-)
