[WIARC] parts needed

I have a project in mind for the club station. It would be handy to have a
convenient way to use multiple headphones from a single source that will
allow independent volume adjustment as well as using both mono and stereo
headphones. What I have in mind is passive, for the sake of simplicity.
I have a circuit but am looking for some parts to build it with. If anyone
has any of the following parts to are willing to donate to the cause, let me
know and I will make arrangements to get them from you.

1 - 1/4 inch (3.5 MM) stereo mic plug (right angle preferred but straight is
OK)
2 - 1/4 inch (3.5 mm) panel mount stereo mic receptacles
2 - 1/8 inch (2.5 mm) panel mount stereo mic receptacles
2 - 5K ohm 2 watt panel mount pots
2 - single pole single throw panel mount toggle switches
2 - 100 ohm 1/2 watt resistors
3 - .01 uF caps, 50 VDC
