Attn:   All Field Day Participants and Volunteers

As required by our Park Permit for Field Day, all participants and volunteers are to sign the attached form releasing the City from any liability for personal injury or death in any way related to participating in the event.

We can do it 3 ways;

1. Print the attached, complete, scan and email to me... or drop off original at the ARES table on your first visit.
2. Email me that you agree and I will and have a copy at the ARES table ready for you to sign.
3. Blank forms will be available at the ARES table.
