[PARC] *Corrected* PARC Activities Survey

I apologize to the club however surveymonkey has changed their billing
policies in order to see more than 14 responses i have to pay over $1000 a
year, that will simply not do.  I have updated the link to a google forms
survey. https://forms.gle/yczwvpTqFJxWGLuc8

I am able to access these results so you can fill out the new form or not
if you choose not to that's fine I can re enter them in the new platform.

Hello All;
With COVID restrictions finally gone, we are looking to create additional
activities, at the end of this year and of course next year. If you could
please take the time to fill the survey out, it will help me with planning
future and current activities.

Please only fill it out if you are a fully paid member in good standing for
2023 (this year), although non members are welcome at our events, we need
to plan based on the membership.

For our members with accessibility issues if it is difficult to fill out
please send me an email and I will arrange to fill it out for you over the
phone.

Please fill it out before September 26th so we have time to present the
information at the October meeting.
