One of the requirements from the Recreation Department in order for TVARC to use the Eisenhower Recreation Center for Field Day is that EVERYONE who participates MUST complete and sign the attached VOLUNTEER INFORMATION SHEET.  Yes, I know that this form looks like an application for a job, but it is used as a release form.  It will save you time if you will print the form, fill it out in advance, and bring it with you to Field Day on the FIRST day that you show up.  No form, no participate.

Don’t bother filling out the hours available.  Just fill out your name, address and emergency contact information and sign the bottom of the first page, including your Villages ID#.  Then read the second page and sign it at the bottom.  Easy peasy!
