Troop 139 Parents:

PLEASE remember to bring to tonights meeting...
-- 3 copies of the attached form (4 pages) signed by a doctor
-- 3 copies of your insurance cards.

Both adults & scouts attending summer camp must give them to me tonight.

We have been emailing and discussing this form since April.  If you have
not handed in your forms, I must have them.  Your child can not go to camp
without it.

I currently only have forms for the following scouts:
Sergio
Joe
Edward
Kevin
Sean
John R
Sam
Reinier
