Hi all, 
 
Attached is the PDF of the Medical Form. Also there is a link if you are unable to download the attachment. 
 
Note: Scouts and scouters that attended the National Jamboree 2013, please bring in 3 copies of your Jambo form. 
     -  please change the fields, however where it mentioned your troop and scoutmaster information
  
Please:

Be sure to bring in 3 sets of this form as soon as possible!  Keep ONE set for yourself. 
Be sure that your scouts Medical exam is within ONE year of the summer camp date. 
That your form is completed IN FULL (High Adventure portion too) 
You have included a copy of the front and back of your medical insurance card (if you have one).
