Hi all, I posted the Camp Medical Forms on our fundraiser page. 
If you need a copy, please download and print from here. I need these within the next 2 weeks.

I will need 3 copies with a copy of your insurance card if you have one. 
All adults AND scouts attending camp or floodwood MUST have a Medical Form completed!
