[TMRA] Please re-input foxhunt survey questions (explained)

For those that already completed the survey, thank you!  However, I had a
setup error in the survey (it turns out unless you make one question
mandatory, it won't record anything).  Therefore, if you could please
re-enter your answers, I'd appreciate it.  Thanks in advance!  - Rob (KV8P)

TMRA had such a great foxhunt last weekend and we are looking to increase
participation for the event as we think it is a very worthwhile skill!
 (Thanks to Brad WB8RG for being the fox for the last hunt!)  For those
that were not able to make the last couple foxhunts, we'd like to know
"why"?
