We had a BAM member do a demonstration on enameling a couple years ago.  It 
was mainly to add a a little color to your forged work, not refurbish 
cookware.  The main idea was to get it clean, use the right flux, and bring 
it up to the temp to get the glass to flow. On an old piece it would be 
tricky to heat it slow enough that the existing enamel doesn't flake off. 
You would probably need a gas forge or heat treat oven big enough for the 
whole piece.

I've seen new enamelware for sale recently, but can't think of the source 
right now.
