I'm sure glad you're only banged up Ron. Falls hurt a lot more for us growd 
up folk.

Safety caps are kind of weird that way, they can be hard to screw on or off 
AND rattle loose at the worse time. Making the threads match too well would 
I assume have less knowledgeable folk oiling the threads and that can be 
B-A-D.

You might try what I do teaching, do a demo then pull up my comfy stool and 
kibitz, correct and sip a beverage while the student works.
