I took off a finger in a table saw many years ago (3/4" dado blade, took 
the whole knuckle, Dr rebuilt it with parts from my foot).   Working 
late at night (early morning) and not thinking things through.  I used 
to think that I would never get hurt because I was too damned afraid of 
the machines to let me guard down.   Now I know that what gets you hurt 
is thinking "I will never get hurt because..."   Even worse is going to 
work thinking "It is only a matter of time"   You generally get hurt 
because you told yourself in one way or another that it was OK to check 
out mentally and not think things through.   Boredom and fatigue kill 
like carbon monoxide.  You don't sense them, you avoid them with 
discipline and planning.
