Great question, thought provoking.  What immediately comes to mind is a 
bent bar holdfast used in a platten.  I would find out what size tubing 
fits into what size hole in a standard platten.  See Acorn Iron works, 
Philadelphia or Weld Sale brand sold by Cunningham Equipment in Phila 
and York PA.  My guess is the tubing or stock used to hold down is too 
small for the hardy hole thus not wedged into the hole enough.  Guessing.
