Now, now, you should never worry about forms. Fill them out
completely, but with whatever comes to mind. If you have a few brews
first you can honestly say you did not >intentionally< lie!
A friend in college (now a popular college professor) was Mexican, but
spent many formative years in the US, including a stint at Hollywood
HS. He asserted that Americans always want you to fill out forms, but
never read them. When asked on a form for his father's profession, he
invariably wrote, "Inventor of black thread." He never was called on
it.
