ANDY,
What it sounded like this AM was that someone should have been
monitoring his muddydikations and there was nothing in place to do that. 
It is the type of thing that there is really nothing to do about it. If he
had tried and failed he would be a danger "to himself or others" and
could have been given a guardian.
It is unfortunate, but I do not think making everyone taking head muddss
have a guardian would be acceptable either.
The guy in Green Bay who sold him some guns on line evidently followed
the rules, which require delivery through a brick and mortar store which
does a local check.
Sometimes the system just flops. That will happen no matter what is set up.
