There are several kinds of inserts that set flush with the floor. One
kind I've used that worked well is called "RedHeads" (because the
wedge/plug that locks it in is red. Duh...).

One tip that won't be news to those who've done a bunch of these but
was an annoying discovery for me when installing a power hammer: Use a
hammer drill (or, possibly, a masonry hole saw) to make the holes, not
just a rotary masonry drill.  The non-hammering masonry drill works
fine if there is no coarse aggregate in the way of your hole. But if
there is a piece of hard rock where you want the hole to go, the
rotary drill will tend to drift sideways, make a non-round and/or
non-plumb hole and leave you with an insecure anchor.  The hammer
drill will gradually fracture the pieces of coarse aggregate rather
than try to drift around it and be more likely to leave a suitable
hole and secure anchor.

And some kind of plastic caps to cover the hole when there's no bolt
in it is desirable (so as to exclude detritus and later problems
re-installing the bolts), if only the end users can be persuaded to
bother with them.
