There should be a thrust bearing, top and bottom between the two guide
wheels, top and bottom. Adjusting these is part of "setting up" the HF saws;
the cutting takes place between the two sets of bearings. These saws come in
almost kit form. Try adjusting them out toward you, and make sure they are
straight up & down. Note: *THIS IS A STATIC ADJUSTMENT*.

Unfortunately, there isn't any flange on either of the wheels, which  
is probably why the blade keeps slipping off. I'll look to see if the  
wheels can be tilted or adjusted the way you explained, but even the  
wheels themselves don't have an obvious angle to them, which most  
likely would have helped the blade stay on.
