That "proper" distance from the ground to the knuckles is only a 
starting point. There are many variables to consider, such as the 
condition of ones back or eyesight. My anvils have always been set 
higher than the measure you suggest. 

That brings up the standard height of the anvil discussion.  I have my anvil
at the standard knuckle height.  And I like it.  But in ALL the pictures of
me hammering, I am hunkered over and it appears to be too low.  The next
time that I get a chance I am going to put something under the anvil to
raise it up a bit until I don't hunker over.

Hmm.  Maybe my arms are too long or something.  At least I don't have
"bowler's arm<G>."
