I just found the issue of The Anvil's Ring with the threading article... 
It's Volume 6, Number 5, from December 1978. The artical is on p. 9, 
"Forging a Screw". It's basically about what I described earlier, taking 
a 1/4" piece of square rod, folding it in half, and wrapping it around a 
larger rod. When you're done you have two helixes.

Improvements:
    Forge the threads into a half-round swage block that (more-or-less) 
matches the threads.
    If you don't have a welder, braze the threads onto the rod... Use 
some flux and copper or brass wire, line the wire along the joint with 
the threads, wrap the whole thing in a wad of clay (to prevent 
oxidation, push a little hole into the clay, and then stick the end into 
the forge.

Jeesh, I do web stuff for a living, you'd think it wouldn't take so long...)
