Been there.

Some vises have a bolt on the end of the shaft, where the handle goes through, so that the handle can be locked in one position and not drop.  Now, if that were a knurled-headed bolt with a rubber tip (opposite the head), then it could be loosely tightened to put a little friction on the handle, so the d****d thing would drop and getcha.

Another idea would be to bend the handle into a very slight sine-wave or helix.  The idea is that it wouldn't drop straight from one end to the other.  The bends would force it to rub up against the hole and slow it down, while not preventing it from being moved from one end to the other.

done that. It had a ball and still gave me a nasty blood blister.
